Parent Authorization Form

I, the undersigned parent or legal guardian, hereby grant my child permission to participate in The Prevention Studio 2026 Creative Intensive Residency, a free, 6-week youth media and podcasting program operating June 13 – July 18, 2026, with a virtual orientation on June 13, 2026 (link sent prior), and ongoing monthly activities thereafter.

I understand that this program is funded by the Georgia Opioid Crisis Abatement Trust (GOCAT) and is produced by PILLARS, with select sessions taking place at the Clayton County Library System as a host venue. I confirm that my child meets the eligibility requirements (ages 14–16, currently enrolled in high school).

I understand that program activities may include discussions of opioid misuse, substance use prevention, and mental wellness as part of the educational curriculum, and I grant my full permission for my child to participate in those discussions.

A significant portion of the program will take place at the Clayton County Library. When sessions require travel to additional shooting locations — which may include PILLARS headquarters (possibly in Stone Mountain) and other selected production sites in the greater area — transportation assistance will be coordinated and provided for participating students.

Final shooting locations will be confirmed and communicated to parents in advance of any off-site session.

Student work produced in this program — including podcast episodes, audio recordings, video content, and written materials — may be published and distributed for public community impact. This includes but is not limited to: podcast platforms, social media, digital advertising, local broadcast spots, community bulletin boards, bus stop advertisements, and billboard campaigns across Clayton County and the greater metro Atlanta area.

Student names and first-name credits may appear alongside published content unless you request otherwise in writing to studio@beapillar.org.
